Your role will be pivotal in evaluating, treating, and coordinating comprehensive care for elderly patients, all while emphasizing quality outcomes and personalized wellness.

*** $60,000 Sign-on Bonus ***

Your total compensation package (base pay + bonus) could exceed $300K, depending on experience and location.

Your Role and Responsibilities

As a Primary Care Physician, you will:

• Evaluate and treat patients in accordance with established standards of care.
• Monitor patient care quality using available data and chart reviews.
• Coordinate patient services, including specialty referrals and home health care.
• Actively participate in daily huddles with the care team, sharing your medical expertise.
• Foster a cooperative practice environment by maintaining a professional demeanor.
• Complete medical record documentation promptly, optimizing coding specificity.
• Engage in discussions about quality of care and outcome data with leadership.
• Explore growth opportunities for new or existing services within the center.
• Participate in the local primary care “on-call” program as needed.
• Ensure compliance with licensing and accreditation standards.
• Focus 100% of your time on direct patient care, including administrative tasks related to patient care.

What We’re Looking For

Required Qualifications:

• Current and unrestricted medical license or willingness to obtain one in your state.
• Graduate of an accredited MD or DO program.
• Excellent verbal and written communication skills.
• Strong interpersonal skills with colleagues and patients.
• Commitment to the integrated team-based care model.
• Willingness to adapt to a value-based care setting.
• Basic computer skills, including email and EMR.
• Must be screened for TB as part of our health protocols.

Preferred Qualifications:

• Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active and unrestricted DEA license.
• Experience managing a Medicare Advantage panel of patients.
• Knowledge of Medicare guidelines and HEDIS quality indicators.
